What a GP Management Plan does

A GP Management Plan (GPMP) is a formal, documented care plan for people with a chronic or terminal medical condition. It is created by your GP and outlines the conditions being managed, the goals of care, treatments and medications, and services required — including allied health providers.

Under Medicare's Chronic Disease Management framework, a GPMP (or equivalent) gives access to rebated sessions with allied health professionals. Eligible providers include physiotherapists, dietitians, exercise physiologists, podiatrists, psychologists (under specific criteria), and others.

How it works via telehealth

Your GP will discuss your chronic condition(s) during a telehealth consultation, review current medications and supports, and develop a structured plan tailored to your goals. You receive a copy of the plan. Referrals to allied health providers are issued electronically.

Regular reviews are an important part of the plan. Your GP will recommend a review schedule based on your condition and progress.
